Wellness Incentive
Employees, spouses, and retirees covered by the City health insurance plan who complete the requirements will be eligible to receive the incentive.
$200 for employees and retirees
$100 for spouses and retiree spouses
Step 1:
Complete enough tasks to receive at least 175 points.
Note, some task require documentation to be submitted.
Step 2:
Submit your completed form and supporting documentation by one of the following ways:

See the following schedule for submission deadlines and reimbursement dates:
Form Deadline
- Submit requirements by April 17th, 2026 and receive incentive: May 1st, 2026
- Submit requirements by May 29th, 2026 and receive incentive: June 12th, 2026
- Submit requirements by June 26th, 2026 and receive incentive: July 10th, 2026
- Submit requirements by September 4th, 2026 and receive incentive: September 18th, 2026
- Submit requirements by October 2nd, 2026 and receive incentive: October 16th, 2026
- Submit requirements by November 6th, 2026 and receive incentive: November 27th, 2026
